Important
You must pay the applicable fee before lodging the form by post or email.
Payment guidance:
- Search for your organisation's payment details using BPAY and Post Billpay.
- Make the payment and attach a copy of the payment receipt to include with your lodgement.
Send a copy of the signed form by email to lodgement@asic.gov.au.

- The email must include the form number, company name and ACN in the subject line.
- Submit the form in PDF format.
- Apply at least two weeks before the due date of your next annual review fee to avoid having to pay the fee.
