Purpose
Use Form 492 to notify us of a correction to a lodged document that ASIC has processed.
How to lodge this form
Lodge online
You can complete and lodge the form online in the relevant portal.
Note: If you are unable to lodge online, you can submit your form by email.

Postal address
Send a copy of the signed form by email to lodgement@asic.gov.au.
Email lodging details:
- The email must include form number 492 correcting [document number] in the subject line.
- You can use ASIC Connect to search for the document number.
- Submit the form in PDF format.
- Include any supporting documents and evidence in PDF format when lodging with the form.
- The maximum size of emails ASIC will accept is 10 MB.
